Worker fatalities inquest set for Chatham

The joint inquest into the deaths of John Janssens, Michael Maukonen and William Swan will take place in Chatham next month.

Dr. Rick Mann, Regional Supervising Coroner for West Region, London Office, recently made the announcement.

Janssens, 73, died on Jan. 15, 2016; Maukonen, 19, died on June 28, 2016; and Swan, 56, died on May 16, 2017, from injuries sustained from falls while working at various roof construction projects. An inquest is mandatory under the Coroners Act.

The inquest will examine the circumstances surrounding their deaths. The jury may make recommendations aimed at preventing future deaths.

The inquest is expected to last five days, and will hear from approximately 14 witnesses.

The inquest will begin at 9:30 a.m. on April 8, at the Ontario Court of Justice, on Grand Avenue West in Chatham.
